Welcome to Young Harris College Athletics. This letter is to inform you of the required materials that must be on file in the training room before participation. Young Harris College carries an insurance policy on all participating athletes. This policy only serves as excess coverage; your family’s personal insurance is the primary coverage in the event that you should be injured during official practices or competition.

As part of our insurance policy, all student-athletes at Young Harris College are required to have their own primary insurance coverage since ours is an excess policy only. If you currently do not own a primary major medical policy that covers intercollegiate athletic injuries or one that meets the college's minimum standards, one must be purchased before the athlete will be permitted to practice or compete. The College requires that your primary insurance policy have a maximum deductible of $1,000 and a coinsurance of 80/20 up to $5,000.

If at any time there is a change in primary insurance coverage, it is your responsibility to inform YHC’s head athletic trainer and submit a copy of the front and back of the new insurance card to the training room. Any lapse in your primary coverage will result in denial of benefits by Young Harris College as well. Expenses incurred for the treatment of an injury will then become your sole responsibility.

To be eligible to participate in Young Harris College athletics you will need to submit the following forms to the head athletic trainer (address below). These forms must be on file in the training room no later than August 1st. This is required so that insurance benefits can be verified before your arrival on campus.

1) Current physical form
2) Copy of current insurance card (front & back)
*Make sure the primary policy holders SS# is listed for insurance verification purposes
3) Athlete’s personal information form
4) Signed copy of YHC insurance policies and claims procedures form
*Both athlete and parent must sign
5) HIPPA consent form
*Needed for insurance verification purposes

The signed copy of the YHC insurance policies and claims procedures form is mandatory and has nothing to do with your acceptance of the student insurance through the college. The student insurance offered through the college does not cover intercollegiate sports injuries.
