Insurance Policies and Procedures

 

Young Harris College Athletic Insurance Policy

1. All student-athletes are required to be covered under a primary major medical insurance policy that covers intercollegiate sports injuries.
2. The athlete’s primary insurance plan must meet these minimum requirements. The deductible must be no more than $1,000 and the coinsurance must be 80/20 up to $5,000.
3. The athlete must carrier a primary insurance card while attending Young Harris College.
4. Non-referred treatment is NOT covered; therefore all injuries must be brought to the attention of the head athletic trainer for referral.
5. In the event of an emergency referral is not necessary. However, if follow-up treatment is needed the head athletic trainer must be notified.
6. All injuries requiring medical care in the community must be reported to and approved by the athletic training staff immediately. Failure to do so will result in denial of benefits.
7. In the event of an injury the athlete or parent/guardian must first file a claim with their primary insurance company.
8. It is the responsibility of the athlete and parent/guardian to be familiar with their primary medical plan. Proper procedures must be followed to assure there will be no reduction in benefits. This includes but is not limited to second opinions, pre-certification, and the use of preferred providers.
9. If the athlete’s primary insurance coverage is provided through an HMO, PPO, POS or any plan using a preferred provider network, a preferred health care provider within the plans network must treat the injured athlete, except in the case of an emergency. Claims denied by the primary carrier for failure to follow their procedures and/or failure to seek care from a preferred provider will result in a denial of benefits by Young Harris College.
10. Although the athletic training staff or insurance coordinator may help with accessing health care in the community it is ultimate your responsibility to make sure your insurance plan(s) rules are being followed (i.e. using preferred providers).
11. Primary insurance deductibles and coinsurance are covered benefits, but not co-payments per-visit. Prescriptions will be covered only if they are deemed necessary for treatment of an athletic related injury.
12. Non-athletic related injuries are not covered.
13. Expenses related to general illness or routine dental care, are not covered benefits.
14. Pre-existing injuries or aggravation of pre-existing injuries due to athletic participation are not covered.
15. Braces and orthotic devices are not covered expenses unless they are prescribed by the treating physician for rehabilitation following a covered treatment.
16. Medical care payments are limited to “usual and customary” charges.
17. Young Harris College reserves the right to limit the number of payable visits for chiropractic and physical therapy care.
18. Charges not covered by the athlete’s primary insurance or Young Harris College become the sole responsibility of the athlete and parent/guardian.
19. All charges must be submitted to Young Harris College within one year of date of service.
20. If at anytime the athlete’s primary medical insurance coverage lapses the College will not offer any coverage for bills incurred related to participation in the intercollegiate sports program.

 

Payment of Medical Bills and Claims Procedures


  1. All injuries must first be brought to the attention of the head athletic trainer, except in the case of an emergency.
  2. The head athletic trainer must approve medical care in the community.
  3. The parent/guardian will be contacted at this time to inquire about their primary insurance policy and procedures.
  4. The athlete will then be sent to the appropriate medical provider in the community.  If testing is requested by the physician (e., MRI, X-ray, CT, bone scan, etc.) an insurance approved provider must be used.
  5. The medical provider will be furnished with the athlete's primary insurance information at the time of service.
  6. After the family insurance has made a determination of payment, the athlete or parent/guardian should submit a copy of the itemized bills, along with a copy of the primary insurance's Explanation of Benefits (E.O.B.) showing the amount they paid on the claim, to the head athletic trainer.
  7. All bills and E.O.B.s must be received within one year of the date of service.
