Immunization Information
All students born on or after January 1, 1957 must submit a completed Certificate of Immunization proir to class attendance.
All students who pay a deposit to YHC receive a Certificate of Immunization in their admissions packet.
The Certificate of Immunization can be viewed and downloaded by clicking the adjacent link.
Note: the Certificate of Immunization must be completed by a doctor or health department official.
Required immunizations include:
* Two MMR (Measles, Mumps, Rubella) vaccinations
* Tetanus shot taken within the past 10 years
* Chicken pox immunity - either by history of having the disease, a positive Varicella antibody test, or by vaccine (one dose required if given between the 1st and 13th birthdays or two doses if first dose is given after the 13th birthday.)
Additionally, it is recommended that students consider the Hepatitis B immunization series and meningitis vaccination.

All Students must also complete a TB screening form which is found on the back of the Certificate of Immunization.
No further action is needed if both questions on the form can be answered "No".
A "Yes" response indicates the need for a PPD skin test and any other follow-up deemed necessary by a doctor or health department official.
